Affidavits of Parentage
The Deputy Registrar can provide assistance with filing Affidavits of Parentage for children born out of wedlock. The following items should be brought to your appointment:
- A certified copy of the child’s birth certificate or mother’s copy from the hospital of birth
- Social security cards for mother, father, and child (if applicable)
- Current ID or valid government issued ID (see acceptable IDs listed below)
Accepted forms of ID
- State-issued driver’s license
- State-issued non-driver photo ID
- Passport
- US military ID (with signature)
- Veteran’s ID card
- Tribal enrollment card
- Work authorization card
- Permanent Resident card
