The Connected Care Program (CCP) is a collaborative project between the Cumberland County Department of Public Health and the Cumberland County Department of Social Services to focus on addressing social determinants of health in an effort to improve overall health outcomes and well being for individuals and families.
The program will deploy teams consisting of case managers, public health educators and community social services assistants to individuals and/or families who are experiencing health problems, food or housing insecurity, unmet social or emotional needs and unemployment. There will be a nurse and nutritionist available to each team for consultation.
Each team will provide case management and care coordination services in person (both in the home or in an office setting) by telephone or virtually. Services will include follow up on healthcare discharge plans, coordination of public health and/or community resources for healthcare, housing, education and employment readiness. Assistance in applying for food and nutrition benefits (food stamps), Medicaid, child care subsidies and other assistance programs at the Department of Social Services. Families and individuals will also receive support in the implementation of case management plans to achieve desired outcomes identified by program participants.
The Connected Care Program will work with individuals and families to the program until the overall health and well being of the family or individual referred to the program has improved or stabilized.
Individuals and families residing in Cumberland County are eligible for Connected Care Program services.
